What it is and how it works

Dermal fillers are injectable gels, most commonly based on hyaluronic acid, that can be placed strategically beneath the skin to restore volume, support facial contours and improve facial balance. Rather than simply “filling lines”, modern dermal filler treatment can be used to consider the face as a whole. Depending on your anatomy and treatment goals, filler may be used to restore age-related volume loss, improve projection or definition, soften transitions between facial areas and enhance symmetry and proportion. At Palmeira Clinic, dermal filler treatment is personalised. The aim is not to change your face or create an artificial appearance, but to identify where carefully placed volume or structural support could create a more balanced result.

What it may help with

• Restores facial volume where appropriate
• Enhances facial contours and definition
• Can improve facial symmetry and proportion
• Can provide structural support to selected areas
• Can soften the appearance of some folds and shadows
• Can replace volume lost as the face ages
• Treatment can be highly targeted and individualised
• Results are visible immediately, although swelling may initially affect the appearance
• Can be combined with other treatments as part of a broader facial rejuvenation plan

Suitable for

Dermal fillers may be suitable for adults who would like to restore age-related facial volume, enhance particular facial features or improve facial proportions and symmetry. They may be considered for areas such as the cheeks, chin, jawline, temples and selected facial folds, depending on individual anatomy. Not every perceived line, hollow or facial change is best treated with filler. Skin quality, muscle activity, underlying structure and existing filler all influence treatment planning. Your consultation therefore considers whether filler is appropriate or whether another treatment, or no treatment, would provide a better result.

Areas treated

Depending on assessment and suitability, dermal filler may be used in areas including:
• Cheeks and mid-face
• Chin
• Jawline
• Temples
• Nasolabial folds
• Marionette area
• Selected facial hollows or areas of volume loss
Lip filler has its own dedicated treatment page because lip treatment requires separate assessment, planning and aftercare considerations.

What happens on the day

Treatment begins with a consultation and facial assessment. We discuss what you would like to change, your previous aesthetic treatments, medical history and whether dermal filler is the most appropriate option. Your face is assessed both at rest and in movement, considering proportions, symmetry, volume distribution and the relationship between different facial areas. If treatment proceeds, the skin is cleansed and filler is placed carefully using a needle, cannula or a combination of techniques depending on the area being treated and the intended result. The type and amount of filler are selected according to the treatment area and your individual anatomy. A staged approach may sometimes be recommended rather than attempting to achieve every change in a single appointment.

Downtime and Aftercare

Temporary swelling, tenderness, redness and bruising can occur following dermal filler treatment. The extent of downtime varies according to the area treated, the amount of product used and the individual. Some results are visible immediately, but swelling can temporarily alter the appearance. Most initial swelling settles over several days, although the treated area can continue to settle for around two weeks. You will receive appropriate aftercare instructions and information about symptoms that require clinical review.

Risks and side effects

Common temporary effects include swelling, bruising, tenderness, redness and localised discomfort. Temporary asymmetry or unevenness can occur while swelling settles. Other possible complications include lumps or irregularities, infection, inflammatory reactions, delayed swelling, nodules, product migration, asymmetry and an unsatisfactory aesthetic result. A rare but serious complication is vascular occlusion, where filler enters or compresses a blood vessel and compromises the blood supply to tissue. In very rare circumstances, vascular complications involving vessels connected to the eye can result in visual impairment. The specific risks vary according to the area being treated. These risks, warning symptoms and the clinic’s approach to managing complications will be discussed before treatment so that you can make an informed decision.

Contraindications

Dermal filler treatment may need to be avoided or postponed in certain circumstances, including active infection or inflammation in or near the treatment area, pregnancy or breastfeeding, known allergy to relevant product components, or certain medical conditions or medications. Previous filler is also important. Existing product, migration, overfilling or previous complications may affect whether further filler is appropriate. In some cases, correction or dissolving of previous hyaluronic acid filler may need to be considered before additional treatment. A full medical history and individual suitability assessment will be completed before treatment.

Concerns this treatment may help

Frequently Asked Questions

They do not have to. An unnatural or overfilled appearance is often associated with inappropriate treatment planning, excessive volume or repeated treatment without reassessing the face. Our approach is to use filler selectively, taking account of your existing features and facial proportions. Sometimes the most appropriate recommendation is to use a small amount, stage treatment over time or not add filler at all.

Dermal filler can be used in several areas of the face, including the cheeks, chin, jawline, temples and selected folds or areas of volume loss. However, whether an area should be treated depends on your anatomy and the cause of the concern. We assess the face as a whole rather than assuming every line or hollow requires filler.

Longevity varies according to the area treated, filler product, amount used and individual factors. As a general guide, hyaluronic acid fillers may last around 6–18 months, although some areas can retain an effect for longer. We prefer to reassess before recommending maintenance rather than treating automatically at a fixed interval.

There is no standard amount that is right for everyone. Treatment is based on your facial anatomy, existing volume, previous filler and the change you would like to achieve. Some patients require only a small, targeted amount, while broader facial rejuvenation may be better approached gradually over more than one appointment.

Many commonly used dermal fillers are made from hyaluronic acid and can be treated with an enzyme called hyaluronidase when clinically appropriate. Dissolving filler is itself a medical procedure with potential risks and is not always necessary. If you have concerns about previous filler, we can assess the area and discuss the most appropriate approach.
